Please complete all of the information below. The entire application must be completed for consideration from the New Mexico Activities Association’s Record Committee. Please only list one record performance per application. If more than one record is set, please include an attached comprehensive listing of performances.

Required along with this application are newspaper accounts of the performance and any other related materials (i.e., scorebooks, etc.) that can help to verify the record performance. Photographs of the performance and/ or the individual/ team are requested.

Please complete this application as soon after the record performance or completion of the season as possible. Applications received more than 30 days after the performance or completion of the season may be too late for inclusion in the next edition of the Record Book.

Performances must be established in competition conducted in compliance with the eligibility rules of the New Mexico Activities Association and all National Federation playing rules.

I submit this application for recognition in the New Mexico Activities Association Records Book. All facts contained herein are duly certified. I realize any inaccuracies could disqualify the mark or performance from recognition. It is also understood that, although the NMAA Records Committee may approve this performance for inclusion in the NMAA Records Book, the passing of time and the continuing improvement in high school athletics might cause the performance to be deleted from the next edition of the Records Book.

I grant to the NMAA the right to publicize this performance in any and all ways it wishes, including through print, broadcast and television media.
